docs(v5): fix neurotransmitter dedup + regenerate all CSV with corrected data

Fixes: deduplicate barrier/scfa/neurotransmitter entries in triplet format (19->11 items per person). Regenerated CSV: antibiotic risk 9 items (was 256), disease risk 15 items, neurotransmitter 49 items (was 79).
